This DX340 faceplate is for AMP12 and AMP14. When using AMP12 with the DX340, the line out works normally. However, the background noise is high when using phone out. Please be aware of this before purchasing. The AMP14 module works normally with the DX340.

DX340 Amp Card Faceplate: dual 3.5mm SE:
This DX340 faceplate is compatible with AMP13.

Great faceplate. A helpful tip:

Wow. Just used the faceplate to update my amp 14 to work with the dx340 - magical. Quick tip to perhaps save you a bit of frustration. There are no instructions included which is not that big of a deal considering the simplicity. However, please note that the included screwdriver in the kit is for removing the amp card from the device not for removing the screws from the plate/card. Those screws are Phillips head screws so you will need your own small eyeglass type screwdriver to remove them.
